You are welcome to print and complete any of the provided forms to bring with you for your initial visit. Each form may be downloaded and filled in electronically, allowing you to input your information directly into the pdf file. Feel free to save your information to the file and email it directly to our office at [email protected]. We will also have forms available for you when you get here, just be sure to come in at least 15 minutes before your appointment time.

Click on the file name to download a copy of the New Client Form. You may print it out and complete to bring with you, type in your information, save, and print to bring with you, or save your information in the form to your computer and email it to [email protected] prior to your first visit.
We have prepared a Referral from Physician for you to provide to your primary care physician. If your doctor wishes to speak with us about your care, we would be happy to do so, and together recommend the best course of treatment. The next time you are in the RuppMassage office in Indianapolis, ask us for a copy of the Prescription / Letter of Referral and a Medical Release Form that allows us to discuss your treatment and health with your physician. You may also email these forms to your primary care doctor to complete.
HSA, FSA and Insurance for Therapeutic Massage
Massage Therapy, particularly for therapeutic massage related to recovery from an injury, rehabilitation, treatment and preventative measures is accepted as an eligible medical care expense by many health insurance plans and is often paid for using Health Reimbursement Account (HRA), Health Savings Account (HSA) or Health Care Flexible Spending Account (FSA). You should check your medical plan’s summary of benefits for more details on what expenses are covered.
RuppMassage office in Indianapolis accepts payment through FSA and HAS credit or debit cards, but we also recommend a prescription from your primary healthcare doctor to ensure your coverage is accepted for therapeutic massage. The prescription should include the following information:
- Medical necessity: why you need massage therapy (example: to relieve back pain)
- Frequency: number of sessions per month (example: minimum of two sessions per month)
- Duration: length of treatment (example: 3,6,9,12 months)
IRS Definition of “Medical Care”
According to the IRS website, the IRS Code, Section 213(d) defines “medical care” expenses as “the costs of diagnosis, cure, mitigation, treatment, or prevention of disease, and the costs for treatments affecting any part or function of the body. Medical care expenses must be primarily to alleviate or prevent a physical or mental defect or illness. They don’t include expenses that are merely beneficial to general health, such as vitamins or a vacation.”
